Christmas Care Package Nomination
Our Christmas Care Packages are sent to children and young people aged 0–25 who are living with a long-term physical and/or mental health condition.
Each package contains a selection of small gifts, treats, and activities to bring a little extra brightness during the festive period, offer some entertainment, and provide something to enjoy during what can be a difficult time.
We recognise that illness affects the whole family, which is why siblings of hospitalised or chronically ill children and young people can be nominated to receive a package too.
Important Information and Eligibility

Children, young people, or siblings must be aged between 0 - 25 and live with, or have a sibling with, a long term physical or mental health condition.

We aim to distribute 100 packages to children and young people in the community in 2026. Nominations will close once all 100 packages have been allocated.

All nominees must be based in the UK. Unfortunately, applications containing a postal address outside the UK will not be considered or acknowledged.

If you are nominating someone else, submitting this form confirms that you have their permission to share their personal information with Cards for Bravery for the purpose of this nomination.

If you are under 16 and nominating yourself, please ask a parent/carer for permission or ask another trusted adult aged 16 or over to complete the form on your behalf.

Any personal information provided through this form will be treated confidentially and used only for the purposes of administering our care package and card scheme and meeting the aims of Cards for Bravery.
Cards for Bravery is the data controller responsible for the personal information supplied about you and the child or young person being nominated. We will not share this information with third-party organisations without obtaining permission, unless we are legally required to do so.
You can withdraw your consent or request the withdrawal of a nomination at any time by contacting contact@cardsforbravery.com.
